Chemical bottle was observed with nozzle at level with a utensil, making it easy to accidentally contaminate utensil. Keep all chemicals away from food contact surfaces. Fix leaking toilet.

Lettuce was stored in container with water. Food must not be stored in direct contact with water. Remove buildup and debris from gasket and prep cooler in general.
