Comments: No employee health policy observed. Implement an employee health/restriction/exclusion policy immediately. This policy will address how to handle sick employees, what they can do and if they can work during their illness. Observed cracked countertops near buffet line. Fix immediately. Observed soiled areas around equipment tops and around splash plates of the wall/floor. Observed deteriorated walls/wall coverings at buffet line, near entrance, in the dining area and throughout the lobby. Areas of tile and wood are broken/rotting in the kitchen as well. The damage to the walls is extensive and is in need of major repairs. Please begin process of fixing the structure immediately. Further deterioration is unacceptable.

Cooler must maintain 41 degrees or cooler. Observed hot foods at buffet held at 122 degrees. Hot holding temperature must be 135 degrees or greater. Pre-heat buffet to proper temperature before use. Observed physical facilities in disrepair, such as metal door frame. Repair items affected.
