Comments: Sanitizer concentration in sani-sink did not register on test strip. Maintain sanitizer level at 100 ppm QAC for sink. All employees must wear hair restraints. Two were observed without restraints. Since a cashier changed duties to food assembly, they too must wear a hair restraint. Both restrooms were observed to have paper towels on floor. One did not have toilet paper. Both had foul smell and broken wall tiles. Supply restrooms and keep clean and in good repair. The tiles by drain and wall of walk-in cooler (on outside) must be repaired as brick material is visible and absorbs water and dirt/food. Facilities must be maintained in good repair to maintain smooth, non-absorbent, easy-to-clean surfaces.
