Comments: A fish fillet was observed on top of a stack of raw ground beef in a container in a walk-in. The fish was removed and discarded. Raw meats must be separated based on cooking temperatures to avoid cross contamination. Educational material provided. Microwave was observed to have some dangerous/peeling plastic on the internal side of door. Repair or replace so that it becomes a smooth, easy-to-clean surface. Multiple pieces of wood throughout the kitchen was observed, used mostly as support. Remove wood or paint it with multiple layers until wood is non-absorbent, smooth and easily cleanable.

Bologna and ham in the top prep cooler were out of temperature and had been out for more than four hours. No time records available. Discarded. Keep all ready-to-eat, potentially hazardous food at 41 degrees or lower or maintain time record if food will be out of temperature. Educational material provided. Corrected on site. Test strips were not available and one employee said they have not had any since the last inspection was done in March. Test strips must be available on site to confirm adequate concentrations of sanitizer.
